  • Email is not a secure way of sending confidential information.

  • If you have pictures or sensitive information to send please do so via SMS text or request a secure link via email.

  • ​Do not send colour pictures of documents, the size will interfere with the import into your chart. Scan in grayscale instead.

  • Please identify yourself in all your interactions with the office so we can locate the appropriate medical chart. If you have no access to email or text, call our office at 902-252-3011

Electronic communications policy: If you prefer not to receive electronic communications (eMail, Ocean) from our office, contact our front desk to have your email address removed from your chart. You will not receive any further appointment reminders or updates regarding specialist referrals.

If we have your email address in your chart we will assume it is your preference to receive electronic communications. Please make sure you are able to receive our emails. Providing your email address while booking an appointment online will automatically add it to your chart.

New Patient Registration Process

If you are contacted by our clinic to be registered as a new patient, please follow the steps below:

  1. Complete Required Forms
    You will receive an email containing a secure link. This link will allow you to complete two required forms:

    • New Patient Registration Form

    • Patient Demographics Form

    Each patient must complete both forms in full.

  2. Book Your Initial Appointment
    Once both forms have been submitted, please go to the ONLINE BOOKING section to book your initial appointment with your doctor.

    • Select the appointment type: Initial Appointment

    • This appointment will be conducted in person

    This visit allows your doctor to review your medical history and become familiar with your care needs.

  3. Future Appointments
    After your initial visit, you will be able to book future appointments either in person or by phone, as appropriate.

Additional Information

  • A family member or caregiver may assist you in completing the forms if needed.

  • If you do not have an email address, are unfamiliar with computers, or are unable to access the internet, you may visit our office to complete the registration forms in person.
